As the largest and most exposed organ of the human body, it is hardly surprising that our skin is also sensitive to damage and change – particularly pigmentary problems. However, pigmentation disorders, especially in skin of colour, can be one of the most challenging conditions we are asked to treat as aesthetic practitioners.

Energy-based devices can be used for the treatment of areas of abnormal hyperpigmentation and uneven skin tone, and many superficial lesions, such as lentigines and café-au-lait macules, can easily be treated with lasers and light-sources. But in many cases, particularly in clients with darker skins, or those with tricky-to-treat conditions such as melasma, pigment can actually darken after treatment if an inflammatory response induces post-inflammatory hyperpigmentation. For this reason, up until now, treatment of conditions such as melasma has generally been addressed using topicals or even systemic medications.

Four modalities

However, the new SmartPico system from Deka, in partnership with Lynton, has revolutionised the treatment of pigment. The SmartPico combines four different emission modalities, pico, nano, double and thermal in two wavelengths, 532nm and 1064nm, making it the most flexible system for the treatment of tattoos and dyschromia; including recalcitrant conditions such melasma and post-inflammatory pigmentation (PIH) in darker skin types.

The high melanin affinity of 532nm makes it the ideal choice for the treatment of superficial pigment in light skins, whereas the longer 1064nm wavelength is the best option for the treatment of dermal pigmented lesions and for skin of colour.

The four separate modalities can be selected to ensure optimal treatment of pigment in all skin types. Picosecond pulses generate a pure photomechanical effect in the skin, and the short pulse duration of 370ps allows the device to deliver exceptionally high powers while still using low fluences. This minimises the thermal effect in the skin, dramatically reducing the risk of post-inflammatory hyperpigmentation, and even allowing treatment of post-inflammatory hyperpigmentation caused by inflammatory conditions or other devices.

Two handpieces

The SmartPico also has two separate fractional handpieces, which can be used for skin rejuvenation, treatment of acne scarring and cutaneous toning treatments. Alongside these, the SmartPico has a unique Deka Peel handpiece for performing carbon peel facials, one of the most soughtafter cosmetic laser treatments worldwide, which results in smoother and brighter-looking skin.
